How to Prepare for MRCPsych Paper A: Syllabus, Study Plan and Exam Strategy

Understand the Paper A format before you plan

MRCPsych Paper A assesses the scientific and theoretical basis of psychiatry. Under the current format, it is a three-hour examination containing 150 questions for 150 marks. Approximately two-thirds of the questions are single-best-answer multiple-choice questions, and one-third are extended matching items.

The exam is divided online into three parts, each containing roughly 50 questions. Candidates can return to questions within the active part, but once a part is completed and a break is taken, it becomes locked. This makes time allocation and final checking within each section important.

There is no negative marking, so every question should be attempted. The pass standard is set for each examination rather than being a fixed percentage announced in advance.

Begin with a diagnostic assessment of your knowledge

Before starting intensive study, complete a short mixed question set without consulting notes. The purpose is not to predict your final result. It is to expose your starting strengths and weaknesses.

Classify each error as a knowledge gap, confusion between similar options, misreading of the stem, weak recall or poor time management. This simple classification prevents you from repeatedly reading material when the real problem is question interpretation or retrieval.

Use a four-stage preparation model

A reliable Paper A plan can be divided into four overlapping stages:

1. Foundation: cover every syllabus domain using one organised primary resource.

2. Consolidation: convert information into active recall, comparison and clinical application.

3. Question integration: complete topic-based and mixed questions while maintaining an error log.

4. Performance phase: sit full-length timed mocks, analyse errors and revise weak areas.

Stage 1: build a structured knowledge base

Paper A contains many facts that are easy to confuse: similar neurotransmitter pathways, receptor actions, developmental theories, statistical terms, adverse-effect profiles and diagnostic concepts. Long narrative notes often make these distinctions harder to see.

Use point-wise notes, tables, diagrams and contrast-based learning. For example, compare dopamine pathways in one table, antipsychotic adverse effects in another and learning theories in a third. Add a clinical example whenever a basic-science concept has a practical psychiatric implication.

Stage 2: convert reading into active recall

Avoid turning every sentence into a flashcard. Excessive card creation consumes time and can fragment understanding. Focus on high-yield distinctions, frequently forgotten facts and errors identified through questions.

Spaced revision is most effective when intervals are planned. Revisit difficult topics after one day, one week, two to three weeks and again during the final revision period.

Stage 3: use questions as a learning tool

Do not postpone question-bank practice until the syllabus is complete. Begin topic-based questions as soon as you finish each unit. Read every explanation, including explanations for answers you selected correctly but were uncertain about.

Keep an error log with five fields: topic, question concept, why the error occurred, correct rule or distinction, and the date to revisit it. The log becomes a personalised revision resource and is often more valuable than rereading entire chapters.

Gradually introduce mixed questions. Mixed practice is cognitively harder but better represents the examination, where the candidate must identify the relevant subject before answering.

Stage 4: practise full examination performance

Full-length mock examinations should be completed under realistic conditions: three hours, limited interruptions, the same break approach planned for exam day and no access to reference materials.

Do not judge a mock only by the score. Review how many errors were caused by knowledge, rushed reading, indecision, section timing or changing a correct answer without a sound reason. Use the findings to design the next week of revision.

At least two or three full mocks are useful, but quality of review matters more than the raw number. A mock that is carefully analysed can change performance; a mock that is simply scored may add little.

High-yield examination technique

  • Read the final line of the stem carefully so you know what is being asked.
  • Identify qualifiers such as most likely, best next step, least likely or strongest association.
  • Answer from the information provided rather than adding assumptions not present in the vignette.
  • When two options appear plausible, compare the exact distinction being tested.
  • Do not spend disproportionate time on one difficult question; mark it and return within the same active section.
  • Attempt every question because there is no negative marking.

The final two weeks

The final phase should focus on retrieval and examination performance rather than learning large new resources. Review your error log, high-yield tables, frequently confused concepts and mock-exam findings.

Complete timed mixed blocks and at least one final full mock early enough to correct problems. During the final 48 hours, reduce the workload, review concise material and protect sleep. Fatigue can impair attention, recall and judgement even when knowledge is adequate.

Important syllabus update for 2027

The Royal College has announced that an updated Paper A and Paper B syllabus will be published in September 2026 and will apply from January 2027. Candidates sitting examinations during 2026 should continue to use the current syllabus. Candidates preparing for a 2027 examination should compare their resources with the new syllabus as soon as it is published.
